As the next monitoring term starts soon,
we are informing you of the monitoring dates from April, 2012 to March, 2013,
and the frequencies and broadcast time for you to monitor from April to October, 2012.

Until now reports from all monitors via e-mails were written in the different formats at monitors’discretion,
however, since we have established the computing system of automatically migrating data of monitor reports to our database table,
we would like to ask our monitors to use the attached Monitoring Form from April, 2012.
We hate to inconvenience you, but we would appreciate your understanding and cooperation
in using the attached common format for your report submission from April.
As for monitoring dates, if the dates in the sheet are not convenient for you due to inevitable reasons,
please monitor on 4 earliest days instead and submit the report as soon as possible.
(Please note that if the report arrives significantly late,
such as after the 7th day of the following month, there may be the cases where the remuneration may not be paid.)
